In accordance with the Privacy Act 2020, the records made from any information submitted may be reviewed on request by you at the school; the information collected may be disclosed to education, health and other specified agencies in accordance with the principles of the Privacy Act; the information will not be disclosed to any other person or agency unless such disclosure is authorised or required by law, or if you specifically authorise the disclosure.
